Bartter syndrome comprises a group of rare autosomal-recessive salt-losing disorders with distinct phenotypes, but one unifying pathophysiology consisting of severe reductions of sodium reabsorption caused by mutations in five genes expressed in the thick ascending limb of Henle, coupled with increased urinary excretion of potassium and hydrogen, which leads to hypokalemic alkalosis. Bartter syndrome type IV, caused by loss-of-function mutations in barttin, a subunit of chloride channel CLC-Kb expressed in the kidney and inner ear, usually occurs in the antenatal-neonatal period. We report an unusual case of late onset presentation of Bartter syndrome IV and mild phenotype in a 20 years-old man who had hypokalemia, deafness, secondary hyperparathyroidism and erythrocytosis. Antenatal Bartter syndrome BS type IV is characterized by polyhydramnios, premature birth, sensorineural deafness, severe salt and water loss, in the perinatal period, hypokalemic alkalosis, fever, vomiting, diarrhea, failure to thrive and chronic renal failure developing during infancy. The patient was the tenth child of consanguineous parents, who born premature by cesarean-section delivery after a complicated pregnancy by polyhydramnios. The patient had other deaf brother and a stone-forming sister.

Osteonecrosis de los maxilares asociada al empleo de bifosfonatos Bisphosphonate-associated osteonecrosis of the jaw. Directory of Open Access Journals Sweden. The main pharmacological effect of bisphosphonates is the inhibition of bone resorption, mediated by a decreased function of osteoclasts without interfering in bone formation and mineralization. These drugs are used worldwide, with clear and clinically proven benefits. Several publications within the last three years consider osteonecrosis of the jaw to be associated with bisphosphonate therapy as a result of their extensive use.
It is important. Background: In late , an increasing number of case reports began to appear on bone exposure of the jaw with a torpid evolution in association with intravenous bisphosphonate use. Bisphosphonates are used to inhibit bone resorption in patients with osteoporosis, multiple myeloma, hypercalcemia of malignancy, lung cancer, breast cancer and prostate.
Among its known causes, besides those of thrombophilia, are viral and bacterial infections, mostly Neisseria meningitidis.
During pregnancy there are alterations in the hemostatic mechanism. However, by themselves, they have not been shown to lead to PF.
The case of a year-old woman who developed PF during pregnancy, presumably due to her Escherichia coli urinary tract infection, is presented including clinical, paraclinical, therapeutic and surgical aspects. She recovered but suffered amputation of the distant phalanges of three toes in the left foot. Osteonecrosis , also known as avascular necrosis, is chiefly characterized by death of bone caused by vascular compromise.
The true incidence of osteonecrosis in HIV-infected patients is not well known and the pathogenesis remains undefined. Hypothetical risk factors peculiar to HIV-infected individuals that might play a role in the pathogenesis of osteonecrosis include the introduction of protease inhibitors and resulting hyperlipidemia, the presence of anticardiolipin antibodies in serum leading to a hypercoagulable state, immune recovery and vasculitis.
Hereby we present a series of 13 HIV-infected patients with osteonecrosis. The most common symptom upon presentation was arthralgia. The majority. Osteonecrosis of the jaw. Osteonecrosis of the jaw ONJ was first reported in the dental literature in The term was coined to describe a spectrum of dental problems seen in cancer patients treated with high doses of intravenous bisphosphonates for the prevention of skeletal-related events.
By consensus, the syndrome is now defined by the presence of exposed bone in the mouth which fails to heal after appropriate intervention over a period of 6 or 8 weeks. It is likely that ONJ results from direct drug toxicity to cells of bone and soft tissue. The bone in ONJ lesions does not appear to be 'frozen' but rather there is very active bone resorption taking place, which is likely to be responsible for the local release at high concentrations of bisphosphonates.
Infection probably plays a pivotal role in driving this resorption, so its active management is critical. Obvious abnormalities are apparent with a variety of radiologic modalities, and it is not clear that radiographs are inferior to other approaches.
Most authors favor a conservative approach to surgical debridement of the lesions. Flora asociada a sitios minados.
A rare cause of osteonecrosis. Full Text Available IntroductionHereditary hemochromatosis HH is an autosomal recessive disorder caused by mutations in the HFE gene, which increase intestinal iron absorption. The prevalence of CY homozygosity, which causes the disorder, is 0. The clinical manifestations are related to excess iron in the tissues, especially the liver, heart, pancreas, pituitary, and skin. They include fatigue, loss of libido or impotence in males, liver disease, skin pigmentation, diabetes mellitus, cardiac enlargement—with or without heart failure, and conduction defects.
It occurs late in the disease, when the total body iron content is more than five times the normal value, about 20 grams. Left untreated, approximately half of all patients with HH eventually develop arthralgia or arthropathy.
Chondrocalcinosis, chronic pseudo-osteoarthritis, and osteoporosis are the major rheumatic manifestations of HH. The cause of the arthropathy is still unknown. Iron deposits within joints may trigger a number of pathologic events, such as free radical generation and crystal deposition, which stimulate immune complex formation and inflammation. Materials and methodsWe describe the case of a year-old male suffering from chronic bilateral ankle pain.
ResultsThe work-up revealed osteonecrosis of ankle. The patient also presented high plasma ferritin levels and homozygosity for the CY mutation. Other than HH, which was confirmed by liver biopsy, the patient had no other risk factors for osteonecrosis. DiscussionHH represents a rare cause of osteonecrosis , and there are no prior reports of aseptic osteonecrosis of the ankle in a patient with this disease.
The pathogenetic mechanism remains unknown.
Alcohol, steroids and cocaine have all been shown to be independent risk factors for osteonecrosis when taken in excess. Here we present a case of a young girl who developed debilitating osteonecrosis secondary to low doses of alcohol, steroids and cocaine. We feel it is important to highlight to those caring for such patients of the potential devastating complication of these three agents. Osteonecrosis of the knee following arthroscopic meniscectomy. Primary osteonecrosis of the knee is characterized by acute onset of pain in elderly individuals who present no risk factors.
Osteonecrosis following arthroscopic surgery for meniscal repair is a rare occurrence, the etiology of which remains to be determined. The authors present the magnetic resonance findings in a new case and a review of the related literature. Author 7 refs. Fueron estudiadas en total muestras de pacientes con histoplasmosis, el El mayor rendimiento se obtuvo con el LCR Seventy four of them presented cryptococcosis, 39 histoplasmosis and 4 both mycoses.
The following specimens were studied for the diagnosis: skin and mucous membrane scrapings, bone marrow aspirations, bronchial secretions, biopsies of different organs, cerebral spinal fluid and blood cultures. Sera were also collected for serologic tests. A total of samples from patients with histoplasmosis were studied, Osteonecrosis or spontaneous fractures following renal transplantation. Determination of metacarpal bone mass at the time of transplantation and registration of bone resorption and soft tissue calcification at the time of transplantation and at the time of onset of osteonecrosis and spontaneous fractures were made.
