The novel is very heavily based on all the ancient accounts of the Julio-Claudian dynasty. But as for how much of that is real "history" or not, well Graves chooses to use all of the worst possible versions of events, the most scurrilous rumors, that these ancient historians recorded.

Lots of backdoor deals, assassinations, breaches of trust, etc. All from the perspective of Claudius, who is an absolute runt amongst many strong, powerful and cunning men and women. I would recommend reading the first fifty pages or so in one sitting if possible though, so that you can remember everything. In the beginning there's quite a lot of names and it was a bit confusing of who married whose ex-wife and such. Don't get discouraged though, it gets much easier from then on to keep track of the characters. Or look up a family tree at the beginning of your reading to clear up any confusion.

There are plenty of debated events where historians make different claims so by experiencing the tv series you'll know most of the plot, its stories etc. Whether you regard that as "spoilers" is somewhat subjective. If you want to read the book, it's probably better to read it first and then watch the tv series. If you don't want to read the book or are ambivalent about it, watch the series and decide later, or read the book after some years where you'll have forgotten much of it.
